For our New Patients:

Our team is here to help collect, review, and process all necessary documention in order to adequately acquire funding/coverage. To get started, simply email us your basic information making sure to include at least 2 phone numbers to contact you.

For our Referral Partners:

Our team is here to help complete the Prior Authorization and procurement of equipment process. Please include most recent visit/progress notes, PT/OT evaluation (if available), things like 485 & Oasis, and anything else that would help us identify the patient and demographics. Documents need to be within the previous 90 days.

Additionally, our team will help with obtaining signatures needed for all documentation.
